Make You Well, First Physicians accepts both traditional Medicare (Original Medicare, Parts A and B) and select Medicare Advantage Plans. Our team is highly familiar with Medicare benefits and works to make your visits as clear and straightforward as possible.

Board-Certified Internal Medicine for Complex Senior Care

For older adults managing complex or multi-system health conditions, Make You Well, First Physicians offers access to board-certified internal medicine through Dr. Darrell S. Parsons, MD, ABIM, at our Westview Medical Clinic location.

Internal medicine is well-suited to the health needs of older adults, who often face conditions that span multiple organ systems or require careful coordination of medications, specialists, and ongoing monitoring. Dr. Parsons brings the depth of training and clinical experience to manage these complexities with precision and care, all within a practice that has known this community for decades.

Chronic Care Management

For eligible Medicare patients living with two or more chronic conditions, we offer Chronic Care Management (CCM), a structured program designed to keep your care coordinated and proactive between office visits.

Chronic Care Management may include:

  • A personalized care plan developed and regularly reviewed by your care team
  • Medication management and reconciliation across your prescriptions
  • Communication and coordination with specialists involved in your care
  • Ongoing monitoring and check-ins to track your health progress between appointments
  • Dedicated support for patients managing conditions such as diabetes, hypertension, COPD, heart disease, kidney disease, and more
Chronic Care Management is a covered Medicare benefit for eligible individuals. Contact your nearest location to learn whether you qualify and how the program works.

Medicare Annual Wellness Visits & Preventive Care

Medicare covers one Annual Wellness Visit per year with a $0 copay. Make You Well, First Physicians strongly encourages all Medicare patients to use this preventative benefit. Your Annual Wellness Visit gives you and your provider dedicated time to:
  • Review and update your complete health history and current medication list
  • Identify recommended preventive screenings based on your age and risk factors
  • Develop or update a personalized prevention plan
  • Discuss any observations or updates regarding your health, memory, mood, or daily functioning

Please note: This visit is separate from a regular sick visit or standard physical. It is designed specifically to support healthy aging and catch potential issues early. If acute illnesses or new conditions are addressed during this preventative visit, standard office visit copays or deductibles may apply.
