The FMCSA requires a DOT physical for any driver who operates a commercial motor vehicle (CMV) in interstate or intrastate commerce, including drivers who operate:

The DOT physical at our Occupational Testing Center is a comprehensive evaluation of a driver’s medical fitness to operate a commercial motor vehicle safely. The exam is conducted in accordance with all FMCSA guidelines and covers the following areas:
Vision
Each driver must demonstrate at least 20/40 visual acuity in each eye, with or without corrective lenses, along with a minimum horizontal field of vision of 70 degrees per eye. Glasses and contacts are permitted.
Hearing
Drivers must be able to perceive a forced whisper at a distance of 5 feet or less, with or without a hearing aid. This standard corresponds to an average hearing loss of less than 40 dB in the better ear.
Blood Pressure and Pulse Rate
Our examiners check your blood pressure and pulse rate to ensure your cardiovascular health is safe for long hauls. To qualify for a standard two-year medical card, your blood pressure must be under 140/90. If your reading falls between 140/90 and 179/109, you may still receive a short-term certificate while working to lower it. Taking prescription blood pressure medication is fully permitted, though it limits your medical card to a one-year maximum renewal cycle.
Urinalysis
A routine urinalysis is performed to screen for kidney issues or signs of unmanaged diabetes (such as excess sugar or protein in the urine). While the FMCSA focuses on overall blood sugar management and stability rather than a single absolute cutoff, keeping your blood sugar stable is key.
Please Note: This baseline health screen is completely separate from a DOT drug test, though using any non-prescribed controlled substances or habit-forming medications is strictly disqualifying under federal law.

The certified medical examiner evaluates all findings, along with the driver’s self-reported health history, to determine whether the driver meets federal medical standards. The determination is made solely by the certified medical examiner.
A DOT medical certificate is generally issued for up to 24 months. Drivers with certain health conditions, such as high blood pressure or diabetes managed by medication, may receive a certificate valid for a shorter duration based on the medical examiner’s assessment. In those cases, more frequent re-examinations are required to maintain an active medical card and remain compliant with FMCSA regulations.
