If you need your records sent directly to an insurance provider, attorney, or an outside specialist clinic, you must complete an official Authorization for Disclosure of Health Information Form.

Under Texas Medical Board rules, healthcare facilities have up to 15 business days to fulfill a valid medical records request after receiving a signed, completed written authorization form. We make every effort to process requests significantly faster, especially for urgent continuity of care needs. If your request is time-sensitive, please note this clearly on your authorization form or contact your clinic location directly.
If you require copies of actual diagnostic films or imaging files — such as digital X-rays from our Richardson or Colleyville offices — please specify this clearly on your release form. Imaging files may be prepared separately from standard progress notes and visit summaries and may require additional processing time.
No. To protect patient confidentiality and comply with HIPAA requirements, we cannot accept medical records requests by telephone. All requests must be submitted through a verified secure channel — either through the Patient Portal or via a signed written authorization form submitted electronically, by mail, or in person.
A family member or designated caregiver may request your records only if they are legally authorized as your Medical Power of Attorney, or if you have submitted a signed authorization form explicitly naming them as an approved recipient. Requests from unauthorized third parties, including family members, will not be fulfilled without proper legal documentation.
If you are a new patient joining our practice, download our Authorization for Disclosure of Health Information Form, list your previous provider as the disclosing party, and select your Make You Well clinic as the recipient. Our team will contact their office directly to retrieve your records. This service is provided at no charge.
In accordance with Texas law, adult patient medical records are retained for a minimum of 10 years from the date of the last treatment. Records for minor patients are retained until the patient reaches the age of 21, or for 10 years from the date of last treatment, whichever is longer. For specific questions about record retention, contact your clinic location directly.
You have the right to request an amendment to your medical records if you believe information is inaccurate or incomplete. Submit a written amendment request through the Patient Portal or by contacting your clinic directly. Your provider will review the request and respond in accordance with HIPAA guidelines. You also have the right to have a statement of disagreement included in your file if your amendment request is denied.
