1. Texas Medical Board Notice

The following notice is provided in accordance with Texas Administrative Code (TAC), Title 22, Part 9, Chapter 178, to inform patients on how to file a complaint regarding a licensed physician, physician assistant, or acupuncturist.

Notice Concerning Complaints

Complaints about physicians, physician assistants, and acupuncturists may be reported for investigation to the Texas Medical Board at the following address:

For clinic locations utilizing Advanced Practice Registered Nurses (APRNs), Nurse Practitioners (NPs), or Registered Nurses (RNs), the following reporting information is provided to patients in accordance with Texas nursing practice regulations.

Notice Concerning Nursing Care

Complaints regarding the practice of professional nursing or advanced practice nursing in the State of Texas may be submitted to the Texas Board of Nursing for review and investigation.
