Daniela Dillman, FNP-C

Founder/ Provider

Daniela Dillman, FNP-C is a board certified Family Nurse Practitioner with over a decade of experience in Urgent Care, Family Medicine, and Aesthetics. She is renowned for her excellent bedside manner, patient communication skills, and strong leadership qualities.

Daniela’s academic credentials include multiple degrees from Loyola Marymount University, Cal State University, and Western University of Health Sciences. She has extensive training in emergency procedures and managing acute and chronic conditions from many years in practice at her offices in Los Angeles and Dallas.

Daniela’s long-time patient population includes Fortune 500 executives, A-list celebrities, and professional athletes. She is a member of the American Association of Nurse Practitioners, California and Texas Association of Nurse Practitioners, and the Urgent Care Association of America.

Daniela is fluent in English, Russian, and Spanish.

Shital Chavda, MD

Medical Director & Provider

Dr. Shital Chavda is a board-certified Family Medicine Physician with over a decade of experience. As a Partner at Make You Well Family Medicine & Aesthetics, she is dedicated to helping patients achieve optimal health and well-being through personalized, compassionate care.

Dr. Chavda earned her Bachelor of Science in Microbiology with minors in Chemistry and Health Sciences from the University of Florida. She went on to earn her Doctor of Medicine from St. George’s University. She completed her residency at St. Luke’s Warren Hospital in Phillipsburg, NJ, where she was appointed Chief Resident, a testament to her leadership and clinical expertise.

Dr. Chavda has been recognized throughout her career for her exceptional bedside manner and strong communication skills. She prioritizes clear explanations of medical conditions and collaborative decision-making, empowering patients to participate actively in their health journey.

Dedicated to lifelong learning, Dr. Chavda is a member of the American Board of Family Medicine and American Academy of Family Physicians. She stays at the forefront of advancements in medicine to provide the highest quality care. Fluent in English and Gujarati, she fosters meaningful connections with a diverse patient population.

Outside of the clinic, Dr. Chavda enjoys traveling with her husband, trying the newest restaurants, playing pickleball and spending time with her family.

Make You Well is a family practice and aesthetics office, meaning we function primarily as a primary care provider offering scheduled appointments for ongoing, preventive, and acute care needs. We do our best to accommodate same-day visits for patients who are not feeling well, making us a convenient alternative to urgent care for established patients. We are not an emergency facility; life-threatening emergencies should go directly to the nearest ER or call 911.

A Family Nurse Practitioner (FNP-C) is an Advanced Practice Registered Nurse with a master’s or doctoral degree and board certification in family medicine. An MD (Doctor of Medicine) and DO (Doctor of Osteopathic Medicine) are physician-level providers. All three can diagnose, treat, prescribe, and manage patient care independently or collaboratively. Make You Well’s team combines FNP-C and MD-level expertise. Daniela Dillman, FNP-C, leads clinical care, supported by Medical Director Dr. Shital Chavda, MD.

Yes. In addition to primary care expertise, our team includes Angela Fulmer, RN, a Master Injector with specialized training in Botox, Dysport, and dermal filler techniques, and Eelan Trejo, our licensed aesthetician. The integration of medical and aesthetic expertise within one practice ensures that aesthetic treatments are performed with the safety standards and clinical oversight of a medical office.

Bring a valid photo ID, your insurance card, a list of current medications, and any prior medical records you’d like your provider to have. New patients are asked to pay at the time of service.
