Chronic Care Management (CCM) is a Medicare-covered program designed to support patients who are living with two or more chronic conditions. It provides structured, ongoing coordination of your care between office visits, including a personalized care plan, medication review, and communication with your broader care team. Your Annual Wellness Visit is a no-cost preventive appointment covered by Medicare each year. It includes a review of your health history and medications, recommended preventive screenings, and the development or update of a personalized prevention plan. It is separate from a standard sick visit or physical exam. We encourage all Medicare patients to schedule their Annual Wellness Visit each year.
